Command Sponsorship

Soldiers requesting Command Sponsorship are required to go through a process prior to submitting the request to the Command.

Family in Country (Newborns):

DA Form 4187 Requesting Command Sponsorship

Endorsement Memorandum Signed by the Battalion Commander

Orders to Germany and any Amendments

Copy of Approved Command Sponsorship

Birth Certificate of Newborn:

• Requesting a Copy of a Birth Certificate, From the States or a Territory
• Requesting a Copy of a German Birth Certificate. Individual will have to go to the Standesamt in the town they are registered in to request a copy.

EFMP Screening -  Family Members are required to be medically screened at Military Treatment Facility for  possible EFMP enrollment. The following forms are required to be filled out by the Military Treatment Facility, DA Form 5888 EFMP screening , and DD Form 2792 and/or DD Form 2792-1. Further information can be found at ACS.

Note:

Soldier’s who have command sponsored dependents and have a child born in country, the child only needs to be enrolled in DEERS, no EFMP needed, the effective date for command sponsorship is the date of birth.

If one of a dual military couple gets an approved overseas separation, the Soldier remaining on active duty must request command sponsorship.

All Documents are required to be in English.

• Marriage Certificate:

Requesting a Copy of your Marriage Certificate, From the States or a Territory

Requesting a Copy of your German Marriage Certificate. Individual will have to go to the Standesamt in the town they are registered in to request a copy.

Birth Certificate of children (if any)

• Requesting a Copy of a Birth Certificate, From the States or Territory
• Requesting a Copy of a German Birth Certificate. Individual will have to go to the Standesamt in the town they are registered in to request a copy.

Custody paperwork for any stepchildren being command sponsored.
